OCI Card Application on Ancestral Basis

Ancestor’s Documents–obtained from MGI (Moka) Archives Mauritius or National Archives Mauritius (1 year validity)

Passport of spouse (if applicable)

Dual Nationality holder – to bring both passports

2 photographs – 50x50 mm, white background compulsory

Note:

Birth/marriage certificate issued by any other country except Mauritius, has to be apostilled by the concerned authorities.

 

OCI Card Application on Spouse Basis

Joint Affidavit from Supreme Court or Lawyer giving details of applicant’s marriage claiming that applicant is still in marriage relationship with the respective OCI or Indian passport card holder

Dual Nationality holder – to bring both passports

2 photographs – 50x50 mm, white background compulsory

Note: Birth/marriage certificate issued by any other country except Mauritius has to be apostilled by the concerned authorities.

            That ever since our marriage, we have been living together at (address) and that our still subsists as at today.

            Solemnly affirmed by the above named deponents at Supreme Court.
